Purpose
To explain how Peninsula Therapy Hub collects, stores, uses and protects personal information of clients, families, and staff.

Applies to all Peninsula Therapy Hub staff, contractors and clients.
Policy
Collection
We collect only information necessary to provide therapy and meet legal/NDIS obligations: name, contact details, health history, NDIS number, session notes, and billing information.
Information is collected directly from the client/guardian unless impracticable.
Use & Disclosure
Used solely for therapeutic care, billing, service planning, and compliance.
Shared only with consent or where required/permitted by law (e.g., risk of harm, court order).
Storage & Security
Records stored securely (electronic and physical) with access limited to authorised staff.
Retained for seven years or until a child turns 25, as required by the Health Records Act 2001 (Vic).
Access & Correction
Clients/guardians may request access or corrections by contacting info@peninsulatherapyhub.com.au or 0488 110 351.
Requests are processed within 30 days unless legal exceptions apply.
Telehealth
Peninsula Therapy Hub uses encrypted platforms and reasonable safeguards; clients are responsible for privacy on their end.
Complaints
Concerns can be directed to the Peninsula Therapy Hub Director.
If unresolved, contact the Office of the Victorian Information Commissioner or the NDIS Quality and Safeguards Commission.
